The process can be empowering and help you discover what beliefs you hold about medical intervention, and help you to effectively communicate your position. Furthermore, it is a gift for your loved ones. By planning your future healthcare, you take away the burden for your family of making difficult decisions without knowing how you feel.

Step 1 – Take time to learn about medical intervention and life sustaining measures as part of your overall planning process.
Step 2 – Talk to your family and care providers about your wishes, including those you would like to be your ‘voice’ or temporary decision maker.
Step 3 – Document your wishes in your Advance Care Plan, and keep this in a safe and accessible place.
