When was the last time you discussed your healthcare wishes with the people you love?

Most of us avoid conversations about inevitable transitions like aging and death.  We talk about what we have planned for the weekend, for holidays, yet only a small percentage of people have these meaningful conversations about what we would like to happen in event we are need of medical intervention, and unable to speak for ourselves.

Advance Care Planning is a process for planning future healthcare, which you can start at any age and stage of your life.  The process can be empowering and help you discover what beliefs you hold about medical intervention, and help you to effectively communicate your position.  Furthermore, it is a gift for your loved ones.  By planning your future healthcare, you take away the burden for your family of making difficult decisions without knowing how you feel.

Advance Care Planning can be as easy as 1-2-3:

Step 1 – Take time to learn about medical intervention and life sustaining measures as part of your overall planning process.

Step 2 – Talk to your family and care providers about your wishes, including those you would like to be your ‘voice’ or temporary decision maker.

Step 3 – Document your wishes in your Advance Care Plan, and keep this in a safe and accessible place.
