The AGE Program is a holistic life planning system that includes: assessment, portfolio and consultation.
It begins with a 45-point assessment that considers a wide scope of matters in the client’s life relating to health, finance, legal, estate management and funeral planning.
Upon receipt of the assessment, the case manager develops a personalized portfolio that further explores statistics, implications, step to take, tips and tools, as well as a check list of to do’s.
Once completed, the case manager follows up and schedules a meeting with the client for a personalized consultation to review the portfolio.
The AGE Program if often used to prepare for aging, retirement and beyond, but it can also be used when a life changing event happens, such as a death.
